They don't really. That one Noel Hill has is one of like 3 in the world, and their origin is shrouded in mystery. Others are making / have made small instruments, but those are the smallest, I think, and still top-notch concertinas.

Yeah, I've been to 3, actually, all run by Noel Hill... 2003, 2004, and one in Ireland in 2005.

So I do play the concertina, though the frequency of my playing has dramatically dropped to almost 0 since taking up improv.
